Team Assessment - FRESH Essentials

Welcome!

In this assessment, please provide your perspective of the __________ team. Your perspective and responses will contribute and shape a way forward for your team. 

Questions are built using Team Matters F.R.E.S.H. framework which details five essentials: Futurize, Realize, Energize, Systemize and Harmonize. 

Please rate your team by answering four questions per essential. You may find you would rate particular situations or team members higher or lower, please think of the overall score for the team as a whole. 

Your responses will generate an overall score, as well as a score for each of the five essentials. These will be emailed to you directly after completion of the questions.  In the following team discussion, you will be asked to share your scores.  An average of the team will be provided once all team members have completed their inputs. This will reveal the areas of strength and opportunities for development you see for your team. 

Please be honest as this will support you in conversations with your team. 

A team conversation will follow to discuss and determine what and how you will collectively put into action the team’s agreed plans for improving effectiveness and performance.
